Job Summary
The Care Manager plays a crucial role in supporting field staff and also individuals with individuals with intellectual/developmental disabilities. The care manager provides oversight and supervision for staff and clients to ensure that individuals live in a safe and clean environment, provide oversight for staff to ensure staff are meeting expectations, clients meet their goals, assist with training field staff, meeting with support coordinators to ensure implementation of the client's service plan, visiting the client homes on a regular basis, interacting with staff, working with ABA team members, organizing each home to ensure homes meet all standards consistently, developing goals with clients and staff for personal hygiene, daily living activities, etc.

Duties

• Assist with the development and ensure implementation of the individualized care plans that are tailored to each client’s unique situation.
• Coordinate services across multiple agencies, including healthcare, behavioral health, and social services.
• Supervise field staff and provide guidance on best practices in crisis management.
• Maintain accurate documentation of client interactions, progress, and service outcomes.
• Advocate for clients’ rights and ensure they receive appropriate resources and support.
• On call for after hours and weekends is required with rotating schedule with other supervisor staff.

Experience

• Proven experience in child welfare, social work, or a related field is highly desirable.
• Familiarity with behavioral health services and crisis intervention strategies.
• Experience in supervising staff or mentoring peers.
• Background in addiction counseling or related therapeutic practices is beneficial.
• Strong communication skills with the ability to build rapport with diverse populations.
• Ability to work collaboratively with multidisciplinary teams to achieve client goals.

Pay: $22.00 - $23.00 per hour
