General Position Description:
Under general supervision must be able to navigate child and family serving systems and teach/support family members who are involved with the child and family service systems.
Performance Expectations and Competencies
The Family Support Partner completes the following tasks:
- Works oneonone and maintains regular frequent contact with the parent(s)/caregiver(s) and/or youth to provide information and support throughout the care planning process
- Attends meetings like the Child and Family Teams and IEP meetings to assist the parent(s)/caregiver(s) and/or youth in articulating the youth/family’s strengths, needs, and goals to the Wraparound Facilitator and CFT.
- Educates and empowers parents/caregivers and/or youth about how to effectively navigate the childserving systems for themselves and about the existence of informal/community resources available to them and facilitates the parent’s/caregiver’s and youth’s access to these resources.
- Assist families in identifying their current strengths and needs. MORES staff assists with developing transition assets and strategies to address those needs using an array of communitybased services, supports, and system collaborations attending/facilitating Child and Family Team meetings, connecting or reconnecting to formal and informal services and supports in the community, and ensuring systems collaboration.
- Provide psychoeducation about psychological conditions, information about navigating the mental health system, reducing stigma, and overcoming obstacles their child is facing.
Referral and Linkage
- Works directly with the youth and family to implement elements of the PCP.
- Identifies, actively assists the youth and family to obtain, and monitors the delivery of available services including medical, educational, social, therapeutic, or other services;
- Collaborates with the other service providers and state agencies (if involved) on behalf of the youth and families.
- Other duties as assigned by supervisor.

g. Support youth in using crisis/safety plans and the skills within that plan.
Preferred and Minimum Position Qualifications:
- Must have lived experience as a primary caregiver for a child/children who may have/had mental health, substance use disorders, or intellectual/developmental disability or lived experience as an individual who accessed the behavioral health system.
- Experience in navigating any of the child and familyserving systems and teaching family members who are involved with the childfamilyserving systems.
- Hold National Certification in Family Peer Support or is actively working on completing certification (within the next 6 months) and is on track to complete Family Peer Support certification within one year of hire date. Family Partner 101 is part of National Certification Trainings for NC http://www.ffcmh.org/certification
Additional Job Requirements (Licenses, Certifications, Personal Vehicle, etc.):
1. Valid driver’s License
2. 24/7/365 is required for this position which may include availability by phone or response in the community as requested by the supervisor.
3. Travel is required as part of this position and may use company supplied vehicle and/or use of personal vehicle as defined in PCC policy/procedure.
4. Must have the ability to operate a telephone, fax machine, and computer hardware/software.
5. Must have the ability to write in a manner that is legible/readable/understandable.
6. Willingness to work in a high-stress/risk environment that is driven by team performance
7. Transportation to and from work in various environmental conditions 24/7 services are essential services that must operate 24/7/365.
8. Must complete and remain in compliance with the following training/certifications:
a. First Aid/CPR
b. EBPI, Parts A/B
c. Blood Borne Pathogens
d. Medication Administration
e. Mobile Crisis Management
f. Continuing education and training
g. Others as specified by program requirements.
