Client Care Coordinator - HomeWell Care Services


Full-time | $50,000–$60,000 per year + bonuses and growth opportunities

At HomeWell Care Services, we provide non-medical home care to clients throughout Litchfield and Fairfield County.


Our Care Coordinators play a critical role in keeping everything running smoothly—managing care plans, supporting our field staff, and serving as the day-to-day connection between clients, families, and caregivers.



Responsibilities:


Client Intake & Service Planning

  • Visit new clients in their homes to understand their needs and create personalized care plans.

Care Oversight

  • Monitor active cases to ensure quality service.
  • Stay in communication with caregivers and families to address any issues as they arise.

Scheduling & Support

  • Manage caregiver schedules to ensure coverage across all active clients.
  • Handle call-outs, scheduling adjustments, and staff performance concerns as needed.

Client & Family Communication

  • Provide reliable, professional support to clients and their families.
  • Resolve concerns with empathy and efficiency.

Documentation & Compliance

  • Maintain accurate records and ensure compliance with internal policies and external regulations

What We’re Looking For:


Experience: At least 1 year of experience in non-medical home care (required).


Education: A degree in nursing, healthcare, or a related field is helpful but not required.


Skills: Strong communication, time management, and problem-solving skills.


Technology: Comfortable with Microsoft Office and Google Workspace.


Transportation: Valid driver's license and access to a reliable vehicle. This role involves local travel within Fairfield County (mileage is reimbursed).


Personal Qualities: Compassionate, dependable, detail-oriented, and able to multitask with a calm, professional attitude.
