Job Summary
The Care Coordinator plays a vital role in leading and supporting our clinical team. This position involves collaborating with families, healthcare providers, and community resources to create and implement personalized care plans. The ideal candidate will have a strong background in assisted living, nursing, or direct support services, with a passion for enhancing senior living.
Duties
- Maintain individualized care plans tailored to the needs our senior living population.
- Coordinate services and support systems among families, healthcare professionals, and community resources.
- Monitor EMR regarding audits, reports, and etc.
- Provide guidance and support to families regarding behavior management strategies and care techniques.
- Conduct assessments to identify resident needs and recommend appropriate interventions.
- Facilitate communication between administrator and clinical team.
- Stay informed about best practices in dementia care and other relevant fields to enhance service delivery.
- Participate in training sessions related to first aid, behavior management, and direct support techniques. Handle schedules and manage a clinical team.
Experience
- Previous experience working with senior living population.
- Background in nursing, social work, or related fields is highly desirable.
- Familiarity with EMR and managing a clinical team.
- Experience as a Resident Care Coordinator. Capable of being on call
- Experience of running a clinical team, that provides great leadership skills.
