REMOTE (WFH) PRN POSITION - SUPPORTING FAMILY CAREGIVERS

We are expecting this role to require minimal hours for the first half of 2025, and may increase over time.

This job is primarily remote (work from home), with in frequent in-person visits to members' homes as needed.

Responsibilities

List as a RN on company license and paperwork
As directed, provide ongoing coaching, support and guidance to caregivers, developing trust, building confidence and reinforcing best practices to influence change and reduce caregiver burnout
Validate the completion of notes on a daily basis and follow up with caregivers based on submissions or lack thereof
As directed, conduct home visits and phone calls, following care plans and evaluating needs and progress towards goals

Requirements

Must have at least two (2) years of experience working with elders or people with disabilities (family caregiver support a big plus)
Ability to be responsive and flexible with your time. You may be called infrequently (during business hours) for administrative support on an as needed basis
Knowledge of community resources related to medical, housing, financial, and social issues for client care
Excellent communication and interpersonal skills. Ability to handle stressful situations with empathy and professionalism
Ability to drive within assigned geographic area; valid driver’s license and automobile insurance is required
