Position Overview
We are seeking a compassionate and organized Care Coordinator to join our attendant care team. The ideal candidate will be bilingual (Swahili/English fluent), have prior experience as a CNA or caregiver, be available to work on Saturdays, and demonstrate proficiency with schedule boards, incident reporting, and in-home visits. This position is essential for supporting our diverse client population and ensuring compliance with the Home and Community Based (HCB) Waiver and Medicaid requirements in Kentucky.
Key Responsibilities
- Coordinate and manage care plans for clients, ensuring their needs are met efficiently.
- Communicate with clients and families in both Spanish and English to provide updates, answer questions, and offer support.
- Collaborate with healthcare professionals, service providers, and community resources.
- Conduct client assessments, schedule appointments, and facilitate referrals.
- Maintain accurate documentation and case notes, including the use of schedule boards to track and manage client visits and staff assignments.
- Complete incident reporting during inhome visits in accordance with HCB Waiver and Kentucky Medicaid guidelines, ensuring timely and accurate documentation of all incidents, including critical and noncritical events[10][11].
- Advocate for clients’ needs and assist with transitions between care settings.
- Ensure compliance with all relevant state and federal regulations, including confidentiality and mandatory reporting requirements.
Qualifications
- Bilingual: Fluent in both Spanish and English (verbal and written).
- Experience: Minimum of 2 year as a Certified Nursing Assistant (CNA) or in a direct caregiving role.
- Schedule Boards: Demonstrated experience using schedule boards or similar tools for managing staff/client schedules.
- Incident Reporting: Familiarity with incident reporting protocols for inhome care, especially those required by the HCB Waiver and Medicaid in Kentucky[10][11].
- Availability: Must be available to work on Saturdays.
- Strong organizational and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate, patient, respectful, kind, empathetic and culturally sensitive.
Preferred Skills
- Experience with electronic health records and digital scheduling systems.
- Knowledge of community resources in Kentucky.
- Experience working with diverse populations.
