MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Job Title: Intake Coordinator
Location: Brockton and Attleboro centers
Job Type: Full-Time. Rate: $20-22 per hour

We're looking for a dedicated Intake Coordinator to join our team and help individuals access the services they need. Responsibilities:

  • Manage and accept referrals for services.

  • Communicate with clinical and operational teams to facilitate appropriate placement based on individual needs.

  • Administer and track the referral process from start to finish.

  • Collect, verify, and process intake demographic, insurance/payment, and referral data.

  • Create and maintain new individual records and ensure intake paperwork is complete.

  • Accurately input data and identify any issues that may delay documentation or placement.

  • Secure and verify funding or insurance coverage.

  • Complete all reporting in accordance with company and regulatory guidelines.

  • Assist others by applying knowledge of policies, procedures, and available programs.

  • Support emergency coverage and crisis intervention when needed.

  • Collaborate with the team to resolve admission barriers.

  • Build community relationships, organize events, and promote services to healthcare professionals.

Qualifications:

  • Bachelor’s Degree in Human Services or related field.

  • Two years of experience preferred (comparable experience/education combinations considered).

  • Strong multitasking, communication, and organizational skills.

  • Detail-oriented and self-motivated.

  • Comfortable engaging with community partners and professional audiences.

  • Passion for helping others and supporting organizational mission and values.
