The TERROS GMH/SA Discharge Planners serve as a liaison between the inpatient
setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient
Providers to review presenting clinical information, identify resources for successful discharge to the
community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient
provider. Discharge Planner activities will be performed in both inpatient and outpatient settings. The Discharge
Planner will connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment
upon discharge. The Discharge Planner will work with BHRs while in the hospital, will work with transitioning
BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as
clinically indicated to ensure effective transition to Outpatient or community services, reduce the risk of
readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and Terros psychiatric provider.
