InterCommunity, Inc is a FQHC LA (Federally Qualified Health Center Look Alike). Our health center provides same-day primary care services to every member of the community, regardless of life situation or ability to pay. We also provide a wide array of behavioral health services. Our community Health Care Centers are located at 281 Main St. East Hartford, 40 Coventry St. Hartford and 828 Sullivan Ave., South Windsor.

InterCommunity’s Addiction Services offers a similarly wide range of substance use treatment and recovery-supportive services, with many sites in the Hartford area. We provide services across the spectrum of care, including primary care, residential detoxification and substance use treatment, multiple levels of residential care, outpatient substance use disorder and mental health services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services and social rehabilitation.

  • Escort patients to exam room, perform vital signs, blood sugar testing, EKG’s, urinalysis, phlebotomy, spirometry etc. as instructed by provider.
  • Ensures lab results are reviewed and signed by provider and disseminated as needed.

Summary:

Works with people in recovery, staff of InterCommunity and other internal and external providers (Such as Probation, Bail Commission and Family Services) in order to promote, support and coordinate services needed to better function in the community, thus improving their quality of life.

Essential Duties & Responsibilities:

  • Provides initial outreach and engagement to CSSD, (and other Judicial prn)
  • Acts as point of contact for CSSD, (and other judicial prn) referrals and working closely with OPEH team, referrals sources and clients, to support and facilitate all facets of client engagement. Actively supports client participation in clinical services, monitoring client’s progress, and the impact of these services
  • Provides assistance and support for the individual in crisis situations and when in need of referral to higher level of care. Supervise Urine Analysis on same gendered CSSD clients completing all appropriate chain of command documentation.
  • Provides linkage with community resources i.e. outside clinical, medical, social, educational, rehabilitative, vocational and/ or other services not offered or unavailing within our delivery systems. Completes all required documentation related to treatment in accordance with established procedures;
  • Complete all required reporting standards to various service providers in accordance with timelines set forth in CSSD program standards
  • Monitor and maintain compliance with the CSSD contractual obligations, including reporting processes, data entry into CDCS, and maintaining appropriate documentation in electronic medical record
  • Participates in CSSD related training
  • Performs related duties as required

Schedule:

Monday, Tuesday Wednesday 8-4:30. Friday and Saturday 10-6pm

*All agency staff are required to attend all mandatory department/agency meetings and trainings*

  • All Support Staff Employees are required to show proof of a CPR Certification within 60 days of official start date

Requirements:

Education &/Or Experience:

A Bachelor’s degree, Also requires experience working in a Recovery-Oriented System of Care.
