Overview:
Plans, organizes and evaluates all functions relative to Social Services. Identifies residents’ social needs and through inter-relationships with all disciplines institutes appropriate plan. Holds responsibility for all Admissions, Transfers, and Discharges.
REQUIRED EDUCATION/EXPERIENCE:
Licensure by the State of Maryland Board of Social Work Examiners required. Prior Geriatric or Long Term Care experience preferred. Has attended and will continue to attend continuing education seminars and workshops to keep abreast of current trends and practices.
REQUIRED SKILLS:
1. Ability to plan, organize, perform, and evaluate all areas of Social Services.
2. Ability to effectively assess all residents following the prescribed time frame and to participate in Resident Care Planning conferences.
3. Ability to present social in-service for employees.
4. Ability to use Point Click Care (PCC).
5. Experience in Long Term Care Setting
DUTIES:
A. SOCIAL WORK
1. Develop one to one relationships with residents and family.
2. Perform assessments on each resident in the prescribed time frame.
3. Participate in Care Planning conferences.
4. Utilize family and community resources as needed.
ADMISSIONS
1. Coordinate resident applications for review by Admissions Committee.
2. Communicate with referring agencies, making agency or home visits as necessary.
3. Confer with resident/family on admission to obtain all necessary information.
TRANSFERS
1. Make all necessary arrangements for inter and intra- facility transfers, following
regulation and policy.
DISCHARGES
1. Institute a discharge plan for each resident within the required time frames, and evaluate/update as needed or required by regulation.
2. Make all necessary arrangements for the discharge of residents, utilizing and assisting in making contact with outside resources, where appropriate.
ADMINISTRATIVE
1. Complete and file appropriately, all forms regarding admissions, transfers, and discharges.
2. Maintain Admission log.
3. Maintain Discharge file.
4. Maintain resident files and arrange medical records for permanent filing on discharge.
5. Complete Daily Census Reports.
6. Participate in presenting employee in-service in related topics.
7. Coordinate and assist outside review agencies.
8. Perform duties as delegated by the Administrator.
Job Type: Full-time
Pay: $26.00 - $28.00 per hour
Expected hours: 40 per week
