Job Details:

Responsibilities

Provides a full range of Social Work Services to patients from admission to discharge.
Participates in the interdisciplinary team care process.
Interacts effectively with external agencies in regard to discharge planning issues.
Advocates for the optimal discharge plan for 3-K patients.
Leads therapeutic groups for patients.
Provides clear and comprehensive documentation..
Communicates effectively and with the interdisciplinary team.
Is knowledgeable about the intricacies of local discharge planning options.
Maintains productive relationships with patients.
Is self-directed and able to work independently as needed. .

Requirements:

Masters Degree in Social Work from an accredited college/university
Licensed in NYS as an LMSW/LCSW
Two years experience in a social service agency preferred.
Psychiatric social work experience preferred.
Knowledge of community resources.
Demonstrated leadership skills.

Posted Salary Range: USD $47.70 - USD $47.70 /Hr. :
