Responsibilities
- Provides a full range of Social Work Services to patients from admission to discharge.
- Participates in the interdisciplinary team care process.
- Interacts effectively with external agencies in regard to discharge planning issues.
- Advocates for the optimal discharge plan for 3-K patients.
- Leads therapeutic groups for patients.
- Provides clear and comprehensive documentation..
- Communicates effectively and with the interdisciplinary team.
- Is knowledgeable about the intricacies of local discharge planning options.
- Maintains productive relationships with patients.
- Is self-directed and able to work independently as needed. .
Requirements:
- Masters Degree in Social Work from an accredited college/university
- Licensed in NYS as an LMSW/LCSW
- Two years experience in a social service agency preferred.
- Psychiatric social work experience preferred.
- Knowledge of community resources.
- Demonstrated leadership skills.
