We are seeking a dedicated and organized Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care and support throughout their treatment journey. This position requires strong communication skills, attention to detail, and a background in medical administrative support. We are a growing non-medical home care agency seeking a highly motivated Home Care Coordinator to manage the recruitment of caregivers, coordinate client services, and maintain regulatory compliance. You will play a central role in ensuring our clients receive excellent care and our caregivers are supported and scheduled efficiently.
Duties
Recruitment & Staffing
- Post job ads, screen candidates, and conduct interviews
- Onboard, train, and maintain caregiver files
- Manage and maintain caregiver schedules and shift coverage
- Monitor caregiver performance and satisfaction
Client Relations & Service Coordination
- Schedule appointments, manage patient records, and ensure accurate documentation in accordance with medical office protocols.
- Serve as the primary point of contact for patients, addressing inquiries and providing guidance throughout their care process.
- Assist in developing individualized care plans
- Match caregivers with clients based on compatibility and needs
- Follow up regularly with clients and families to ensure satisfaction
Compliance & Documentation
- Maintain accurate employee and client records
- Ensure compliance with state regulations and company policies (e.g., background checks, trainings, care plans)
- Prepare and maintain timesheets and service documentation for audits
- Assist with billing and service documentation
- Onboard, train, and maintain caregiver files
- Manage and maintain caregiver schedules and shift coverage
- Monitor caregiver performance and satisfaction
- Maintain confidentiality of patient information and adhere to HIPAA regulations at all times.
Requirements
- 2+ years in home care, healthcare admin, or social services (non-medical home care preferred)
- Experience in caregiver hiring and scheduling
- Excellent communication and organizational skills
- Working knowledge of HIPAA and caregiving best practices
- Comfortable with Microsoft Office, Google Workspace, and scheduling software (e.g., AxisCare, ClearCare, or similar)
- Familiarity with Michigan Medicaid waiver programs or VA in-home care
