Community Support Team (CST) provides direct support to adults with a diagnosis of mental illness, substance use, or co- morbid disorder and who have complex and extensive treatment needs. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community.
The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual’s community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal.
Job Duties include:
- Demonstrate use of appropriate knowledge, skills, abilities, and judgement and provide therapeutic services that evidence quality, performance, and meet the needs of persons served
- Show knowledge and use of clinical process to achieve positive outcomes to include: evaluation, assessment, service, treatment planning, intervention techniques, aftercare, and transition planning
- Participate in community based team approach that utilizes the resources of the community, consumer, and treatment team to achieve positive outcomes
- Demonstrate the ability to manage an assigned caseload and meet the direct care billable productivity expectations of the organization
- Maintain accurate service and billing record documentation that clearly communicates the services provided and consumer progress
Education and Experience Required: A bachelor’s degree in Special Education, Psychology, Social Work or closely related Human Services area.
- For AP – must have 1 year of professional work experience with population served
- For QP – must have 2 years of professional work experience with population served.
- Exceptional written, oral, interpersonal, and organizational skills.
- High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint. Or an equivalent combination of education and experience
