RESPONSIBILITIES


The purpose of the Social Worker II position in the Centralized Intake Unit of Aging and Adult Services is to provide the Intake, Education, and Referral function for Aging & Adult Services Programs. Responsibilities include receiving and screening all incoming calls for the division, providing information and referrals to internal programs or external community resources, and completing intake for Adult Protective Services, Guardianship, Meals on Wheels, In-Home Aide, Special Assistance In-Home, Group Care Monitoring, and the Low-Income Homeowner’s Relief Program.

KNOWLEDGE, SKILLS & ABILITIES


  • Ability to utilize appropriate intervention modalities, including problem-solving and crisis intervention

  • Excellent customer service skills, including active listening and exceptional verbal and written communication

  • Strong understanding of the dynamics of human behavior, especially in crisis situations

  • The ability to build rapport and foster positive interactions with indivuduals & families, internal teams, and community partners

  • In-depth knowledge of community resources and service programs within the Durham metropolitan area

  • Comprehensive understanding of social work case management principles, techniques, and practice


MINIMUM REQUIREMENTS


Bachelor's degree in social work from an appropriately accredited institution; bachelor's degree in a human services field from an appropriately accredited institution and one year of directly related experience; bachelor's degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
