Position Description:

The Care Manager is responsible for coordinating client care in collaboration with licensed mental health clinicians. Duties include but are not limited to:

Essential Functions and Responsibilities:

  • Assists children/teens and their families suffering from persistent mental illness improve overall functioning in daily life by providing support, education and coordination with community resources.
  • Assists with developing treatment plans and objectives in collaboration with the mental health clinicians.
  • Works closely with clinical and support staff to insure a smooth and timely process of information gathering, decision making and scheduling of assigned clients.
  • Serves as the first point of contact for all requests from current clients through telephone contact or in person with the requesting party. Gathers all necessary information so as to come to an understanding of the appropriate level and need for services per established protocols. Accurately records information gathered.
  • Uses effective communication skills in interacting with patients and families so as to ensure a smooth, timely and patient-focused treatment experience.
  • Utilizes clinical staff as appropriate in assessing and responding to potential safety concerns that may arise from client follow-up and care-management.
  • Based on clinical assessment, treatment plan and provider direction, refers patients to appropriate care providers and community resources.
  • Assists in scheduling appointments for external resources as needed.
  • Assists parents and others in creatively identifying and eliminating any barriers that may exist to keeping scheduled appointments or from following treatment recommendations.


Experience and Education:

  • Bachelor's degree in Human Services or related field required
  • Minimum of 1 year of previous experience in behavioral health setting preferred


Skills and Abilities:

  • Superior verbal and written communication skills. Strong interpersonal skills so as to maintain strong, positive relationship with clients and team members.
  • Demonstrated strong and creative problem solving skills.
  • Ability to remain calm and reassuring in tense situations and to de-escalate crisis/urgent situations.
  • Strong organizational skills.
  • Thorough understanding of applicable ethical codes and standards.
  • Thorough understanding of compliance measures related to HIPPA and patient health information.
