Job Title: Home Health & Hospice Intake Specialist
Location: Caregivers Home Health Services (CHHS)
Job Summary:
Caregivers Home Health Services (CHHS) is seeking a highly organized and detail-oriented Intake Specialist to join our team. The Intake Specialist plays a critical role in onboarding new patients by processing referrals, verifying insurance coverage, coordinating care authorizations, and ensuring timely initiation of home health services. This position requires excellent communication skills, a strong understanding of healthcare processes, and the ability to work collaboratively with referral sources, patients, and internal clinical teams.
Key Responsibilities:
- Receive and review incoming home health referrals from hospitals, physicians, discharge planners, and other healthcare providers.
- Verify patient insurance eligibility, coverage, and benefits for home health services, including Medicare, Medicaid, and private insurers.
- Obtain prior authorizations when required and ensure all documentation complies with payer and regulatory requirements.
- Accurately input patient data, referral information, and clinical documentation into the Electronic Health Record (EHR) system.
- Communicate with referral sources, patients, and caregivers to clarify orders and confirm readiness for services.
- Coordinate with the clinical team to schedule start-of-care visits and ensure timely admission.
- Monitor referral workflow to avoid delays in patient care and escalate issues as needed.
- Maintain HIPAA compliance and protect patient confidentiality in all interactions and documentation.
- Collaborate with the billing department to ensure correct insurance data entry and support proper claim submission.
- Keep up-to-date with home health compliance standards, Medicare/Medicaid guidelines, and payer requirements.
Preferred Qualifications:

- High school diploma or equivalent required; associate’s or bachelor’s degree preferred.
- Experience in a home health, hospice, or healthcare intake/referral setting preferred.
- Knowledge of insurance verification, authorizations, and home health eligibility criteria.
- Familiarity with Electronic Health Records (EHR), especially home health platforms like Kinnser, Axxess, or MatrixCare.
- Strong communication, organizational, and customer service skills.
- Ability to multitask and prioritize in a fast-paced, deadline-driven environment.
- Understanding of HIPAA regulations and patient privacy requirements.
Work Environment:
- Full-time, in-office position
- Monday to Friday, 9:00 AM – 5:00 PM
- Extended hours may occasionally be needed based on patient or referral volume
- Requires frequent use of computer systems and phone communication
