We are seeking a Medical Social Worker who works well in a fast-paced setting. In this position, you will assist patients and their families in making psychosocial adaptations specific to their diagnosis regarding mental/behavioral health, while facilitating eligibility determination for social programs and funding resources. You will apply optimal utilization of health care services and community resources in accordance with the principles of human growth and development. The social worker will assess, plan, implement, deliver, monitor, and evaluate the options and services required to address a patient's individualized plan of care in collaboration with the interdisciplinary team to achieve discharge goals.
Tasks.
- Prepares a comprehensive psychosocial assessment of patient and family, including assessment of the illness and environment within 5 days of admission unless family declines.
- Provides patient and family counseling services that are consistent with the established plan of care and medical supervision including bereavement counseling in situations determined to be high risk.
- Secures patient’s and family’s understanding regarding hospice program.
- Ensures that patients and family understand their responsibilities as outlined in the care plan.
- Prepares a social work plan of care and submits it to the primary physician upon initial visit and every 60 days thereafter. or when there is a change in care plans
- Assist patients and families in obtaining aid for social, economic, and emotional problems.
- Refers patient and family to other community agencies as needed.
- Keeps all records and documentation in accordance with agency policies.
- Offers assistance in obtaining third party reimbursement for Hospice services, demonstrating familiarity with private insurance coverage, Medicare home care, Medicare Hospice Benefit, Medicaid, and other programs.
- Participates in developing the patient care plan, including discharge planning as appropriate.
- Collaborate with the members of the Interdisciplinary Team to develop a comprehensive plan of care and goals. The plan of care is reviewed and revised throughout the course of service to assure appropriate response to the patient’s and family’s changing needs.
- Interprets social resources to clinical staff.
- Provides guidance, counsel, and instruction to agency staff regarding the social, economic, and psychological needs of patients and families.
- Provides consultation to agency staff regarding the social and emotional needs of terminally ill patients and families.
- Makes recommendations for interventions in individual cases.
- Provides in-service training to agency staff as directed.
- Abides by the agency policy regarding drug diversion and reports any concerns regarding medication to supervisor immediately.
Required Qualifications:
- Graduate of a school of Social Work approved by the Council of Social Work Education with a master’s degree
- One (1) year experience in a medical facility (hospital, clinic, rehabilitation center, etc.) where the team approach to treatment is utilized
- Academy of Certified Social Workers (ACSW) certification or be in the process of acquiring certification is preferred, but not required
