We are seeking a compassionate, detail-oriented, and experienced Medical Social Worker to join our growing home health team and support the Home Health Agency as we actively prepare for CHAP accreditation. This position is vital to our mission of delivering high-quality, patient-centered care in the home setting. As our agency works toward CHAP accreditation, the MSW will play a key role in upholding clinical and regulatory standards that align with CHAP guidelines and best practices.
Duties of Position:
Provide medical social services to patients in their homes in accordance with physician orders and under the supervision of the Clinical Manager or other appropriate supervisor.
Position Responsibilities include, but are not limited to:
- Help the medical team to understand the social and emotional factors affecting the patient and his or her family.
- Help the patient and his or her family to understand the medical team's activities.
- Assist in the admission process of the patient to the Agency by performing and initial evaluation, assessing the patient's psychosocial status, and evaluating the patient, family, and home to identify socioeconomic and emotional factors that will affect the plan of treatment.
- Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to medical social work.
- Observe, record, and report changes in the patient's emotional and social factors that affect the patient's illness and his/her need for care and his/her response to treatment.
- Consult with the attending physician concerning alteration of the plan of treatment.
- Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan, and daily notes.
- Evaluate the patients and family's response to, and effectiveness of, the medical social work intervention.
- Participate in Agency activities and committees when appropriate to include staff development activities and in-service education.
- Participate in discharge planning.
Job Conditions/Qualifications
- The ability to communicate well, both verbally and in writing in English.
- The ability to access patients' homes which may not be routinely wheelchair accessible required.
- Master's Degree from a School of Social Work approved by the Council of Social Work Education.
- Must be licensed in accordance with the Nevada Revised Statutes (NRS) 641B.220
- Two years' experience preferred, with at least one year experience in a health care setting (hospital, clinic, rehab center, etc.)
- Must pass criminal background check.
- Must have current CPR certification.
