About the Job:

Imagine stepping into a welcoming primary care clinic, ready to provide vital mental health support within an integrated, interdisciplinary team. As our Mental Health Professional, you'll work collaboratively as a patient advocate, assisting individuals in chronic disease management through therapeutic interventions, ongoing education, and seamless care coordination. This is a full-time, 40-hour-per-week, benefit-eligible role based onsite in Providence, RI or Louisville, KY. You'll serve employees of PPL Corporation and its affiliate companies, with services extended to client locations across Kentucky, Pennsylvania, and Rhode Island. While not required at the time of application, you will need to obtain full licensing in multiple states (RI, PA, KY, & IN) as soon as possible upon acceptance of an employment offer.

What You’ll Do:

  • Provide therapy using evidence-based therapeutic modalities to help patients process and identify personal stressors related to relationships, family, parenting issues, trauma, mental illness, addiction, or abuse.
  • Assist with crisis navigation and assessments.
  • Explore coping skills and new ways of focusing on strengths to improve emotional wellness.
  • Assistance with short-term leave and FMLA documentation related to mental health.
  • Assist patients in making lasting changes to improve their overall health using therapeutic interventions.
  • Serve as a facilitator for patient continuity of care, acting as a conduit between health center services and community resources.
  • Provide psychoeducation to patients and employers.
  • Identify patients' Social Determinants of Health needs and address barriers by referring to community resources, RX navigation, or helping patients understand insurance benefits.
  • Occasional travel (PA, RI, KY) for engagement opportunities for mental health presentations.

What You’ll Bring to the Team:

  • Education: M.S.W. (Master of Social Work) or equivalent degree from a graduate school of social work fully accredited by the Council on Social Work Education (CSWE).
  • Licensure: Current License with the Board of Social Work (LCSW, LPC, LMFT) or equivalent license in a related field, and in good standing.
  • Experience:
    • At least 12 months of experience (within the last 24 months) assessing and providing evidence-based individual counseling within a mental health or healthcare facility.
    • At least 12 months of direct hands-on experience (within the last 24 months) with computer operations, basic word processing, data entry, and use of an automated medical record system.
    • Recent experience (within the past 3 years) providing integrated behavioral health services within a primary care setting is preferred.
