Our Electrophysiology department is dedicated in providing cutting-edge care and innovative treatments for patients with heart rhythm disorders. We are at the forefront of innovation, offering the latest in electrophysiological procedures and therapies. Our services include catheter ablation, implantation of pacemakers and defibrillators, and cardiac resynchronization therapy. The Care Coordinator - RN works to maximize the patient’s health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator – RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families).
What you will do as an RN Care Coordinator:
  • Coordinates care across the health care continuum within designated sub-specialty population(s)
  • Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services
  • Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits
  • Facilitates proactive disease management and preventative health care using evidence-based practice
  • Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:
  • Current Minnesota RN license
  • BLS certification
  • Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy

Hours: 32 hours a week
