Make a Difference in Patients' Lives
As a Social Worker at US Renal Care, you play a vital role in a dedicated team focused on empowering patients with kidney disease to thrive socially and emotionally throughout their dialysis journey.
Your Responsibilities
- Advocacy & Support: Collaborate with an interdisciplinary team to ensure patients receive exceptional care. Conduct thorough patient assessments to understand their psychosocial needs and develop tailored care plans. You will recognize and address psychosocial issues while providing vital education to patients and families. Actively coordinate support networks and connect patients to community resources, financial assistance, and appropriate agencies.
- Team Collaboration: Foster a culture of teamwork within the clinic. Educate staff on psychosocial care and participate in continuing education and team meetings. Work closely with the Medical Director, physicians, and maintain strong partnerships with local agencies and community organizations.
- Safety & Quality Improvement: Contribute to clinical and operational processes to enhance patient health outcomes and reduce hospitalizations. Engage in Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with all regulatory standards.
What We Seek
- Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
- Current licensure in good standing for the applicable state, except in AZ, PA, or Guam, with all necessary practice requirements met.
- Strong command of the English language with effective verbal and written communication skills.
- Basic proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Ability to master USRC clinical applications within 90 days of hire.
Preferred Qualifications
- Experience providing social services to dialysis patients is highly desirable.

- Must meet relevant state practice requirements.
- Additional licensing requirements may apply based on state regulations.
