PACE Home Care Coordinator (HHA)
Salary: $52,062.40 annually
Schedule: Monday – Friday, 8 AM – 5 PM

Position Summary
As a key member of the Interdisciplinary Team (IDT) and under the supervision of the PACE Home Care Coordination RN Manager, the PACE Home Care Coordinator HHA supports the coordination of unskilled home-based care services for San Diego PACE participants.
You will conduct home safety evaluations, coordinate meal delivery, caregiver services, durable medical equipment (DME), and ensure services are delivered in a person-centered, compassionate manner.
Key Responsibilities
Conduct home safety evaluations (pre-enrollment, annual, semi-annual, and change of address).
Coordinate IDT-approved caregiver services, meal delivery, and DME/incontinence supplies.
Monitor and document service delivery in participant electronic medical records (EMR).
Review progress notes and address any gaps in services.
Advocate for participant needs while ensuring quality and timely care.
Collaborate with caregiver agencies, DME suppliers, and interdisciplinary team members.
Conduct monthly visits for homebound and RCFE participants.
Assist participants with surveys and deliver supplies as needed.
Minimum Qualifications
✔ Certified Nursing Assistant (CNA)
✔ California Department of Public Health Home Health Aide (HHA) certificate
✔ CPR & First Aid certification (American Heart Association)
✔ Valid California Driver’s License with reliable transportation
✔ At least 1 year of experience working with the frail or elderly population
✔ Basic computer skills (Microsoft Outlook, Excel)
Preferred Skills
Strong organizational and communication skills
Ability to work collaboratively in a team-oriented healthcare environment
Familiarity with PACE programs and geriatric care
Compliance & Requirements
- Must pass pre-employment background check, physical exam, and reference checks
- Completion of I-9, new hire orientation, and annual health screenings (PPD compliance)
- Adherence to HIPAA & OSHA guidelines
- Compliance with mandatory vaccinations & boosters as a condition of employment
