We are currently looking for an LPN or CMA to join our Grand Forks team as a Clinical Care Coordinator. This position is responsible for assisting Nurse Practitioners and Physician Assistants care for senior patients in Assisted Living and home environments. You will be an advocate for the patient's needs by collaborating with both the patient and their care team to ensure that the patient is supported throughout their healthcare journey.
