Job Description

Provides counseling, advocacy, education and chemical dependency treatment services to patients.

Job Responsibility

  • Obtains data from patients/families to assess spiritual needs; coordinates the provision of pastoral care support services to Hospice patients and families.
  • Formulates and documents in the medical record an individualized plan of care for patients in a timely manner, in compliance with departmental guidelines.
  • Conducts individual, group and family counseling sessions.
  • Serves as a liaison to community Clergy to ensure that Hospice patients and families who are involved with an existing religious institution are having their spiritual needs met.
  • Develops a network of pastoral care support (priests, nuns, chaplains, ministers, rabbis, et.al) to provide spiritual care to meet the need of patients and families of all religious backgrounds.
  • Serves as a resource in ethical decision-making; plans and/or conducts educational programs, memorial services, bereavement programs in keeping with the Hospice mission.
  • Guides pastoral care students or interns who are doing field placements or practice with the Hospice program.
  • Participates in the various community activities to promote recognition of the Hospice program and its philosophy.
  • Participate in continuing education and in-service training as required.
  • Operates under general guidance and work assignments are varied and require interpretation and independent decisions on course of action.

Job Qualification

  • Bachelor's Degree required, or equivalent combination of education and related experience.
  • Current Association of Professional Chaplains Certification, or Current National Association of Catholic Chaplains Certification, or Current Neshama: Association of Jewish Chaplains Certification required, plus specialized certifications as needed.
  • 1-3 years of relevant experience, required.
