Location: Honolulu, HI

Pay: $85,000/year

Experience: At least 1 year of experience in a related social work field.

Education: Master’s Degree in Social Work (MSW); Current Hawaii LSW license required.

Type: Full-time; Direct Hire

Schedule: Monday – Friday, 8:00 AM to 4:30 PM

Kahu Malama Nurses is seeking a Licensed Social Worker to join a growing and dynamic team!

Job Description:

  • Conduct comprehensive bio-psycho-socio-cultural assessments and develop therapeutic treatment plans for patients and families.
  • Provide individual, couple, and group therapy addressing issues such as grief, adjustment to illness, family dynamics, and mental health.
  • Offer crisis intervention and supportive counseling for end-of-life care, including decision-making and advanced directives.
  • Facilitate case management and referrals to appropriate community resources.
  • Collaborate closely with a transdisciplinary care team to create and adjust patient-centered care plans.
  • Participate in regular interdisciplinary group (IDG) conferences and clinical supervision.
  • Educate and support patients and caregivers on disease processes, care planning, and community services.
  • Assist in training and onboarding new staff.
  • Maintain accurate clinical documentation and reports.
  • Uphold ethical standards and contribute to a compassionate, mission-driven care environment.

Position Requirements:

  • Licensed Social Worker (LSW) in the State of Hawaii.
  • Proficient in assessment, treatment planning, therapy techniques, and case management.
  • Knowledge of end-of-life issues and hospice social work practices.
  • Familiarity with community resources and healthcare systems.
  • Effective communication and interpersonal skills.
  • Strong organizational skills and ability to manage multiple priorities.
