Kahu Malama Nurses is seeking a Licensed Social Worker to join a growing and dynamic team!
Job Description:
- Conduct comprehensive bio-psycho-socio-cultural assessments and develop therapeutic treatment plans for patients and families.
- Provide individual, couple, and group therapy addressing issues such as grief, adjustment to illness, family dynamics, and mental health.
- Offer crisis intervention and supportive counseling for end-of-life care, including decision-making and advanced directives.
- Facilitate case management and referrals to appropriate community resources.
- Collaborate closely with a transdisciplinary care team to create and adjust patient-centered care plans.
- Participate in regular interdisciplinary group (IDG) conferences and clinical supervision.
- Educate and support patients and caregivers on disease processes, care planning, and community services.
- Assist in training and onboarding new staff.
- Maintain accurate clinical documentation and reports.
- Uphold ethical standards and contribute to a compassionate, mission-driven care environment.
- Licensed Social Worker (LSW) in the State of Hawaii.
- Proficient in assessment, treatment planning, therapy techniques, and case management.
- Knowledge of end-of-life issues and hospice social work practices.
- Familiarity with community resources and healthcare systems.
- Effective communication and interpersonal skills.
- Strong organizational skills and ability to manage multiple priorities.
