- Working primarily in the school environment, will demonstrate excellent outreach and engagement skills, forming positive relationships with school staff, students, and parents.
- Conduct comprehensive diagnostic assessments for referred students, incorporating input from students, parents and educational staff. These will be conducted at a location convenient for the family, including school, office or home.
- Develop treatment plans to meet assessed needs, including specific goals, objective and interventions.
- Facilitate referrals to other Signature Health services and community resources as needed.
- Work with school staff to plan and implement early intervention activities, including screenings, classroom presentations, and discussion/ student support groups.
- Assist clients to learn and utilize behavior and feeling management techniques interchangeable within a school and home setting.
- Participate in school team meetings; MFE and IEP meetings, both at the individual student level and for purposes of developing effective building strategies to meet student needs.
- Maintain client charting in accordance with Medicaid, JCAHO, ODADAS, ODMH, CARF and professional standards. All documentation will be entered into the agency Electronic Medical Record.
- Maintain clear communication with referral sources or designated liaison individuals in the school and community.
- Assist in community outreach.
- Reach out and engage parents, providing assistance in implementing effective parenting interventions.
- Act as an advocate/liaison for client and parents with school and other community partners/agencies
- Comply with all agency policies and procedures.
- Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.
- May be required to perform telehealth services as determined by school, family preference and Signature Health protocols.
- Adherence and completion of compliance training provided by Signature Health.
- Responsible for and completes all productivity/documentation requirements.
- Participate in all assigned staff meetings and staff development programs.
- Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.
- Other duties as assigned.
- Bachelor's Degree required, Master's Degree preferred.
- Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or MFT License in the state of Ohio required.
- American Heart Association (AHA) Basic Life Support (BLS) certification required.
- Possess an understanding of children's development, parenting, and family systems
- Work is normally performed the school setting, but may occur in office or in client home as needed.
- While hours of operation are generally aligned with school schedule, position may require flexibility to work evenings and extended hours to meet family needs may be required.
- Requires periods of sitting, standing, telephone, and computer work.
- Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.
- Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.
- Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.
- Possible exposure to blood borne pathogens while performing job duties.
- Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.
- Sufficient dexterity to operate a PC and other office equipment.
