Job Title: Lead Care Manager
Location: Servicing Contra Costa County
Schedule: Monday – Friday, 8:00 AM – 3:00 PM
Compensation: $25 – $30/hour DOE
About the Role
We are seeking a compassionate and driven Lead Care Manager to join our team and support the CalAIM Enhanced Care Management (ECM) program. This role focuses on providing whole-person care through outreach, education, care coordination, and support services. As a Lead Care Manager, you will work directly with enrolled members—primarily in their homes—to help them navigate complex healthcare systems and address social determinants of health.
Key Responsibilities
- Interview potential members and review eligibility data for enrollment in ECM.
- Coordinate with healthcare providers, community services, and other partners to ensure seamless care and non-duplication of services.
- Complete comprehensive assessments, develop and update individualized Care Plans, and provide ongoing education and care coordination.
- Routinely check in with assigned members via home visits, phone, or telehealth; accompany members to appointments as needed.
- Document all member encounters, care coordination efforts, and progress notes in the Electronic Health Record (EHR) system in a timely and accurate manner.
- Monitor and follow up on referrals to ensure continuity of care and that member needs are being met.
- Identify and respond to concerns involving member safety, pain, or mental health; create safety plans as needed.
- Maintain knowledge of local resources (housing, food, transportation, financial support) to connect members with appropriate services.
- Comply with mandated reporting laws related to child and elder abuse.
Required Qualifications
- Medical Assistant (MA) or Certified Nursing Assistant (CNA) diploma (required for compliance).
- Minimum 2 years of experience in a healthcare-related role.
- Current and valid Driver’s License, clean driving record, and proof of auto insurance.
- Ability to travel throughout the county to conduct home visits (90% of work is in the field).
- Strong computer skills including Microsoft Office, email, and virtual platforms.
- Excellent communication and interpersonal skills.
- Ability to work independently, prioritize tasks, and follow through on responsibilities.
Preferred Qualifications
- Bilingual in Spanish and English (verbal and written) is a strong plus.
- Experience with care coordination, social work, or community health programs.
