**Job Summary:**

Valor Hospice is seeking a dedicated and compassionate Hospice Intake Coordinator to join our team. The ideal candidate will be responsible for receiving and processing hospice eligibility for patients requiring our services. This role involves working closely with Medicare, Medicaid, Blue Cross Blue Shield (BCBS), and requires familiarity with 3071, 3074, and TMHP to ensure seamless patient care and compliance with all regulations. Additionally, the Hospice Intake Coordinator will manage patient record filing and maintain accurate documentation of all intake processes.

**Key Responsibilities:**

- Receive and evaluate requests for hospice services from patients and healthcare providers.

- Conduct eligibility assessments for hospice services to determine patient qualifications.

- Liaise with Medicare, Medicaid, BCBS, and TMHP to confirm coverage and address any billing inquiries.

- Utilize forms 3071 and 3074 as part of the eligibility and intake process.

- Maintain accurate and organized patient records, ensuring compliance with HIPAA regulations.

- File and manage patient documentation efficiently and securely.

- Collaborate with interdisciplinary teams to facilitate patient admission and care coordination.

- Address inquiries from patients and families regarding hospice services and eligibility.

- Assist in the continuous improvement of intake processes and patient care services.

**Qualifications:**

- High school diploma or equivalent; Associate’s degree in healthcare administration or related field preferred.

- Prior experience in a healthcare setting, preferably in hospice or palliative care.

- Familiarity with working with 3071, 3074 forms, TMHP, and knowledge of Medicare, Medicaid, and BCBS regulations and processes.

- Strong organizational skills and attention to detail.

- Excellent communication and interpersonal skills, with a compassionate approach to patient care.

- Proficient in using electronic health record (EHR) systems and Microsoft Office Suite.
