• Title: Licensed Social Worker
• Location: Scarborough, ME 04074, United States
• Shift: Days 07:00 - 03:30 (30-minute meal break included)
• Duration: 12-week contract highly possibility of extension
• Weekly Gross Pay: $2,366 for 40 hours

Job Description:

Requirements:

• Must have Maine State Social Worker License.
• BLS is required- Prior to start (can be obtained once position offered)

Preferred:

• 1 year experience as social worker, Previous experience in a SNF, LTC, or geriatric care setting preferred.

Responsibilities:

• Conduct psychosocial assessments for new admissions
• Develop and implement individualized care plans
• Provide counseling to residents and families on adjustment to long-term care
• Coordinate discharge planning and facilitate smooth transitions to home or other facilities
• Serve as a liaison between residents, families, staff, and external agencies
• Advocate for residents' rights and well-being
