Job Title: Home Care Intake Coordinator

Reports To: CEO Designee

Employment Type: Full-Time

Position Summary:

Alpha Comfort Care Seeks an Intake Coordinator who would be responsible for coordinating the admission of new patients to home care services. This role serves as the first point of contact for referrals, gathers necessary patient information, verifies insurance coverage, ensures compliance with regulatory standards, and works closely with clinical and administrative teams to ensure timely and accurate service initiation.

Key Responsibilities:

Receive and process referrals from hospitals, physicians, case managers, and other referral sources
Work collaboratively with Field Marketers.
Collect demographic, clinical, and insurance information accurately and completely
Communicate with patients, families, physicians, and referral sources to coordinate intake and start of care
Verify insurance eligibility and benefits; obtain prior authorizations when required
Maintain compliance with HIPAA and other regulatory guidelines
Collaborate with the clinical team to ensure smooth transition from intake to service delivery
Document intake information in the electronic medical record (EMR) system
Follow up on incomplete referrals and pending documentation
Track and report referral data and trends to management
Provide excellent customer service and respond to all inquiries in a timely and professional manner.

Qualifications:

bachelor’s degree Required
Experience in home care, healthcare intake, medical office, or related field preferred.
Strong knowledge of home care services, insurance processes, and healthcare terminology.
Excellent communication, organization, and time-management skills.
Ability to multitask and work in a fast-paced environment.

Job Type: Full-time

Pay: From $52,000.00 per year
