About the Role
Circle of Care St. Louis is seeking a compassionate and dependable Community Support Specialist to assist older adults and individuals with disabilities as they return home from hospitals or skilled nursing facilities. This hybrid role combines hands-on personal care with light case management and community outreach.
In this role, you’ll provide direct assistance with Activities of Daily Living (ADLs) for at least 20 hours per week. You’ll also support transitions in care, attend wellness visits, and help clients access benefits such as Medicaid or housing resources. This position is ideal for someone with CNA-level experience who is ready to grow into a career in care coordination, case management, or public health.
Key Responsibilities
In-Home Personal Care (Minimum 20 Hours/Week)
- Provide ADL support: bathing, grooming, dressing, toileting, ambulation, and meal prep
- Observe and report changes in client condition or environment
- Reinforce daily routines and safety practices that promote independence
Community Visits & Transitions Support
- Conduct in-home wellness checks to assess safety and unmet needs
- Assist clients with completing benefit applications (e.g., Medicaid, housing, utility support)
- Coordinate with hospitals, SNFs, and family caregivers for smooth discharge and reintegration
Public Health Outreach
- Represent Circle of Care at community events, health fairs, and senior resource expos
- Connect clients to vital community resources such as food pantries, transportation, or chronic disease programs
Documentation & Communication
- Document visits and services in Caresmartz360 and CharityTracker
- Submit notes timely for Medicaid billing and internal reporting
- Maintain regular communication with the case management team regarding client needs and progress
Who We’re Looking For
Minimum Requirements
- CNA certification or at least 2 years of experience in caregiving or personal care
- Reliable transportation, valid driver’s license, and auto insurance
- Comfortable using smartphones, tablets, and digital documentation systems
- Able to pass a Missouri Family Care Safety Registry background check
Preferred Qualifications
- Associate degree in human services, health sciences, or public health
- Experience working in home care, skilled nursing, or care transitions
- Familiarity with public benefits (e.g., Medicaid, SSI, Section 8 housing)
- Bilingual fluency in Spanish, Bosnian, Vietnamese, or another language
