Seeking a full-time Social Worker (LCSW/LICSW required) to support patients on dialysis in an outpatient setting.
- Build meaningful, long-term relationships with patients and their families.
- Help patients manage physical, mental, emotional, and financial aspects of End Stage Renal Disease.
- Work within a supportive, team-oriented environment that values work-life balance.
- Provide patient advocacy, education on rights and responsibilities, and support during healthcare transitions.
