Deliver a comprehensive range of social work services, including psychosocial assessments, treatment planning, therapeutic interventions, crisis support, discharge planning, and resource referrals.
Serve as a key liaison between internal and external care teams to ensure effective coordination and utilization of healthcare resources.
Take a proactive leadership role in guiding patients toward their health goals while achieving optimal clinical, financial, and resource outcomes.

Master's Degree in Social Work from an accredited school
Upon hire: Washington Clinical Independent Social Worker License
1 year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
