UW MEDICINE MONTLAKE PRE KIDNEY TRANSPLANT has an outstanding opportunity for a PATIENT CARE COORDINATOR.

WORK SCHEDULE

Full-time (40 hours per week)

Day Shift

Monday-Friday 8:00 am-4:30 pm

Responsibilities

Coordinates and schedules extensive diagnostic tests necessary for transplant evaluation and coordinates continuum of care for kidney care patients

Interacts with physicians (community and UWMC) and nurses to schedule all diagnostic tests as ordered by physicians in a timely manner

Coordinates scheduled diagnostic tests with clinic visit appointments when possible but, per protocol, does not delay radiology or laboratory testing based on clinic appointment availability

Reports patient concerns to MD/RN regarding their medical, social, financial, psychological or cultural needs as it relates to the planning, personal comfort, or safety of the planned test or procedure

Interacts with patient to discuss itinerary of the diagnostic tests, pre-test patient preparation instructions and answer questions

Orders interpreter and translation of correspondence as needed

Holds responsibility for continuum of care steps following initial kidney transplant appointment until listing and annual updates while on kidney waiting list: diagnostic test, procedure and appointment scheduling, outgoing referrals and coordination of care with PCP/nephrologists' offices, and completion of missing outside provider documentation

Contacts various departments to schedule program tests; completes necessary requisitions and consults at UWMC and/or outside facilities

Prepares schedule for diagnostic tests and clinic, plus corresponds with patients to provide necessary program instructions

Coordinates the receipt of requirements: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians' offices, nephrologists' offices and other medical facilities

  • Receives outside laboratory results on kidney care patients undergoing serial laboratory monitoring per their treatment plan

In preparation for return clinic visits, ensures recent lab work, radiology results, and pathology reports are completed and available

Follows up to ensure exams are completed or rescheduled if necessary and notifies MD/RN of unusual delays in the completion of exams

Assists physicians and nursing staff maintain systems to safely manage patients on the kidney transplant waiting lists, especially updating demographic information and scheduling required tests and appointments

Develops letters to communicate common patient issues that involve compliance and continuity of care

Attends monthly kidney care team meetings and other meetings as required

Works with nurses, physicians, program coordinators, patient services specialists, medical assistants, patient care coordinators and other departmental staff members on various projects

Qualifications

Bachelor's Degree in a medically related or public relations field AND TWO years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology

Three years of Patient Support Service experience
