- Collaborate closely with patients, families, and interdisciplinary healthcare teams to advocate for and address patients' psychosocial needs.
- Deliver a comprehensive range of social work services, including psychosocial assessments, treatment planning, therapeutic interventions, crisis support, discharge planning, and resource referrals.
- Serve as a key liaison between internal and external care teams to ensure effective coordination and utilization of healthcare resources.
- Take a proactive leadership role in guiding patients toward their health goals while achieving optimal clinical, financial, and resource outcomes.
- Uphold the highest standards of ethical practice, guided by the NASW Code of Ethics and aligned with the Swedish Mission and Vision.
- Master's Degree in Social Work from an accredited school
- Upon hire: Washington Clinical Independent Social Worker License
- 1 year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting
