Primary Responsibilities
- Receive and manage new referrals from hospitals, SNFs, physicians, and social workers (phone, email, fax)
- Coordinate patient admissions with the Director of Nursing, case managers, and field staff
- Schedule clinician visits (LVNs, CHHAs, RNs, therapists) and update calendars
- Enter patient data into EMR software (Axxess, Kinnser, WellSky, or similar)
- Communicate with families and patients regarding services, documentation, and updates
- Verify insurance/authorizations and obtain documentation as needed
- Assist with documentation for start-of-care, recertifications, resumptions, discharges, etc.
- Support case managers in day-to-day tasks, follow-ups, and patient progress tracking
- Maintain HIPAA compliance and accurate, up-to-date patient records
Required Qualifications
- Minimum 1 year of experience in home health, hospice, or a medical office setting
- Strong organizational, communication, and multitasking skills
- Proficient in EMR systems (preferred: Axxess, Kinnser, WellSky, or similar)
- Comfortable managing phone calls, referrals, and provider communication
- High school diploma or equivalent required; medical admin certification is a plus
- Basic knowledge of home health compliance and scheduling practices
- Fluent in English; bilingual in Spanish, Armenian, or Tagalog is a plus
- Able to handle fast-paced environment with high attention to detail
- Professional and friendly attitude with patient-centered focus
Job Types: Full-time, Contract
Pay: $22.00 - $28.00 per hour
Expected hours: 35 – 45 per week
