About The Position
Job Title: Social Worker Job Specialty: Social Work - Home Health/Hospice Job Duration: 13 weeks Shift: 12-hour days, Monday-Friday, 8-5 Guaranteed Hours: 40 hours per week Experience: Minimum of 1-2 years License: ND Social Worker License Certifications: BLS, and must hold one of the following: LMSW-AP, MSW, LCSW, CHP-SW Must-Have: - Critical thinking - Flexibility - Knowledge of death and dying - Prefer experience with HomeCare HomeBase EMR Job Description: - Hospice admission assessment and bereavement assessment - Development of plan of care - Follow-up of family and client social needs - May assist with social needs of home health clients - Always have managerial or supervisory support available for consultation - No clinical involvement - Will float between two locations for hospice and home care services - Business casual dress code - No weekend or holiday requirements - Orientation is expedited and based on traveler's needs
