As Community Liaison, you will work directly with the CEO to develop authentic

relationships with community partners, families, and referral sources. You’ll be a trusted

face in the Long Beach / Torrance region, helping to expand the company’s impact while

maintaining its high standards of integrity and compassion.

● Territory familiarity and drive are more important than years of SNF marketing

experience.

Qualifications:

● 1+ year in healthcare or community outreach preferred

● Valid driver’s license and insured vehicle required

● High school diploma or GED required; AA/BA in marketing, business, or

healthcare preferred

● Excellent communication and follow-through skills

● Comfort with technology tools (Google Suite, CRMs, etc.)

● Spanish or other language fluency is a plus but not required

Core Responsibilities:

● Build genuine relationships with case managers, discharge planners, ALFs,

SNFs, and other healthcare professionals

● Help tailor care plans with compassion and precision

● Support clients and families through check-ins and service coordination

● Represent the company’s brand and mission in the community

● Provide operational support as needed to the CEO and care team
