GENERAL SUMMARY:

The Community Counselor provides strength-based interventions to build self-regulation skills and develop positive coping strategies for issues such as anger, impulse control, depression, anxiety, stress, conflict resolution, and self-management. Services are provided in community, educational, or home-based settings. The role also involves care coordination, Child and Family Team (CFT) facilitation, and may include group counseling sessions.



JOB RESPONSIBILITIES:

  • Provides direct family-centered clinical services and care coordination to meet members' needs, including individual, family, and group interventions addressing behavioral challenges, parenting, family systems, anger management, and substance abuse.
  • Prepares written diagnostic assessments, evaluating service recipients from multiple perspectives and using various assessment tools.
  • Facilitates Child and Family Team (CFT) meetings and collaborates with other service providers and professionals.
  • Interprets and follows treatment plans from other providers when appropriate.
  • Responds to crisis situations during the workday as needed.
  • Documents case activities for clinical and billing purposes in accordance with agency and AHCCCS standards.
  • Works with Clinical Supervisor to enhance therapeutic interventions.
  • Performs other duties as assigned related to the position.


QUALIFICATIONS:

  • Associates degree preferred; high school diploma or GED required.
  • Minimum of 4 years of direct member care experience in Behavioral Health.


REGULATORY REQUIREMENTS:

  • Minimum 18 years of age.
  • Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
  • CPR, First Aid & AED certification, if required (must possess upon hire and maintain).
  • Valid Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration, and liability insurance if required.
  • Negative TB test result, if required (provided by employer).
