Position Available:

245D Intake Coordinator

Set up, schedule and represent Placement Partners at intake and/or informational meetings with potential new consumers and their ID team members. Gathers, completes and files all potential intake documents for the corresponding Program Director per 245D licensing requirements. Provide coverage duties as needed.

Minimum Qualifications

Must be at least 18 years old, have a valid driver's license with a reliable vehicle and proof of up to date auto insurance. Must be able to pass DHS Background Check.

Preferred Qualifications

Associates Degree with an emphasis in Social Services, Previous 2-year experience serving in a program under MN statute 245D

Job Type: Full-time

Pay: $21.00 - $23.50 per hour
