Summary/Objective:Primary job responsibilities involve service linkage and care coordination to assist consumers in addressing their behavioral health needs and tracking relevant health outcomes. Case Managers will provide community support, case management and service linkage both within Boundless and with outside providers that identify and address behavioral health needs, developmental issues and barriers to care. The Care Coordinator will work with consumers, parents/guardians, Boundless staff and outside providers that support treatment and contribute meaningfully to whole health outcome of the individual served.Essential Functions:Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Provides case management support to consumers engaged in behavioral health services including community psychiatric support and therapeutic support to promote the coordination and implementation of treatment goalsServe as a liaison between families and agency to positively promote collaborative processes in the treatment planning for clients by encouraging parent/guardian participation in treatment.Identify and assess psychosocial and life skills needs through analysis of factors impinging on client success including factors in the home, school and community.Provide parent/guardian educational workshops on identified issues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication.Keep caseload spreadsheet (information management) updated regularly.Manage phone calls from families, guardians, agencies, etc. Set up meetings; obtain information, etc.Provides case management (coordinate and link all services) for clients and families.Develop programs to address parent/guardian participation in agency services and client engagement in the treatment process.Maintain all required clinical records and submits appropriate documents for statistical reports with adherence to program standards.Locate and mobilize community resources to support the treatment.Attend meetings and professional development activities as required.Provide 22 hours of billable service each week.All other assigned duties.Competencies:Exemplary attendanceExcellent communication skills, both oral and written, demonstrating professionalism with co-workers, parents and consumersExcellent organizational skills with the ability to stay focusedAbility to prioritize multiple tasksAbility to follow all company policies and proceduresDemonstrated clinical knowledge related to behavioral health issues and developmental disabilities.Intermediate computer skillsProfessional telephone etiquetteMust be able to show tact and diplomacy when dealing with parents, clients, peers and staffSupervisory Responsibility:NoneWork Environment:This job operates in a professional office environment. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.Position Type/Expected Hours of Work:This is a full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand.Travel:Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion.Education/Experience:

Bachelor’s degree in psychology, social work, or other related field OR at least three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers.
High School and GED required
Experience in one or more of the following areas of expertise:

Family systems
Community systems and resources
Case management
Child and family counseling or therapy
Child protection
Child development

Additional Eligibility Qualifications:

Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles - No more than 5 points on driving record.
Valid car insurance.
Current High Fidelity Wrap-Around and CANS Assessor training or the ability to complete such within 90 days of hire
Training in cultural competency or the ability to complete such within 90 days of hire.
