About the Role

We are seeking a compassionate and detail-oriented Licensed Social Worker (CSW or LSW) to support residents’ emotional, social, and discharge planning needs. In this role, you’ll work closely with families, outside agencies, and interdisciplinary team members to ensure residents receive high-quality, person-centered care in a skilled nursing facility setting.

Key Responsibilities

• Plan and implement a comprehensive social services program for residents and families

• Complete psychosocial assessments and maintain accurate documentation

• Coordinate discharge planning and community resource referrals

• Conduct family meetings at admission and every 3 months thereafter

• Support residents and families during personal and environmental challenges

• Serve as a liaison between residents, families, external agencies, and the facility

• Assist with Medicaid/financial concerns and provide support when financial status changes

• Document and track social service interactions per regulatory requirements

• Maintain compliance with federal, state, and local regulations

• Participate in facility safety protocols and emergency procedures

• Uphold residents’ rights, dignity, and emotional well-being

Required Qualifications

• Bachelor’s degree in Social Work or a related human services field (e.g., Psychology, Sociology, Rehabilitation Counseling)

• CSW or LSW licensure (New Jersey) required

• Strong understanding of family dynamics and resident advocacy

• Excellent documentation, organizational, and communication skills

• Familiarity with discharge planning, Medicaid, and community services

Preferred Skills

• Knowledge of Skilled Nursing Facility (SNF) operations and discharge regulations

• Experience with EMR systems such as PointClickCare is a plus

• Bilingual (Spanish or Creole) a strong advantage

• Experience with Medicare and Medicaid compliance in long-term care settings

Additional Information

This is a full-time, on-site role located in Pennsauken, New Jersey. Must hold valid CSW or LSW licensure in New Jersey to be considered.
