Key Responsibilities:
As a Social Worker, your role will be pivotal in enhancing our residents' care experience by:
Conducting Resident Assessments: Evaluate residents' psychosocial needs, ensuring personalized care plans that reflect individual preferences.
Providing Counseling & Support: Offer confidential counseling services to residents and families, addressing concerns related to aging, adjustment to nursing home life, and emotional well-being.
Coordinating Care Plans: Collaborate with interdisciplinary teams to ensure a comprehensive, person-centered approach to care.
Discharge Planning: Lead smooth transitions for residents by coordinating discharge plans, ensuring that the transition to home or other care settings is seamless and supported.
Resource Connection: Guide families to community resources, ensuring access to financial support, legal services, and other assistance when needed.
Crisis Intervention: Provide immediate support to residents experiencing emotional distress or psychological challenges, ensuring timely interventions and care.
Qualifications:
Education: Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
Licensure: Must be licensed as a Social Worker (LCSW or equivalent) in the state of NJ
Experience: Minimum of 2 years of experience in a healthcare or long-term care setting, preferably skilled nursing or geriatric care.
Skills & Competencies:

Strong communication and interpersonal skills
Ability to maintain confidentiality and handle sensitive information
Skilled in crisis management and conflict resolution
Knowledge of community resources and long-term care regulations
