Summary:
The social worker screens and assesses psychosocial needs. identifies discharge planning needs; provides information and referrals to patients/families regarding community resources and services; provides supportive counseling; participates in interdisciplinary team staffing; acts as a resource person and documents assessment, intervention and plan. The social worker will also monitor and evaluate the case management program effectiveness and participate in continuous improvement efforts. Patients cared for include all age groups through the infancy to geriatric continuum.

Essential Job Functions:
- Screens and assesses psychosocial needs, anticipated interventions and required resources for high-risk patient/family.
- Acts as a resource person for patient, family, staff, and physician, communicating pertinent information regarding patient's psychosocial and discharge status. Provides supportive counseling to patients and families.
- Provides information and referrals to patients/families regarding resources and services, i.e., financial, equipment, home health, nursing home, etc.
- Participates in formal and informal interdisciplinary staffing regarding patient care and care planning.
- Documents assessment, interventions and plan according to JCAHO standards.
- Identifies discharge planning/teaching needs upon admission. Assists with developing and coordinating discharge plans and arranges post-discharge services as appropriate for patient needs.
- Monitors and evaluates case management program effectiveness.
- Performs other duties as assigned or needed to meet the needs of the department/organization.
- Licensed Baccalaureate Social Worker (LBSW) | North Dakota Board of Social Work Examiners | Prior to Start Date | HR Primary Sources
