Position Type: Full-Time

Schedule: Monday–Friday, 8:00 AM to 4:00 PM, with flexibility to provide services in late afternoons or outside of the school setting if needed.

Overview

Are you an experienced Registered Nurse passionate about youth mental health and community support? We're looking for a School-Based Clinical Care Coordinator (RN) to join our team and deliver compassionate, integrated care directly within a school setting. This role is designed for someone who thrives in multidisciplinary environments and is committed to advancing mental, emotional, and physical well-being for youth.

You’ll be a key connector—working closely with families, educators, mental health providers, and community partners to ensure youth receive the timely, coordinated care they deserve. While primarily based in a school, flexibility to provide care in the community when needed ensures families are supported holistically.


Key Responsibilities

Integrated Youth Care Coordination

  • Assess needs and create tailored care plans for youth and their families
  • Facilitate access to behavioral health services, primary care, educational supports, and social services
  • Guide families through insurance navigation, financial assistance programs, and appointment coordination
  • Manage transitions of care, including after hospitalization or crisis episodes
  • Monitor progress and adapt care plans to maximize effectiveness and engagement

School-Based Clinical Collaboration

  • Partner with school counselors, educators, and mental health staff to address challenges impacting youth well-being
  • Participate in school-based team meetings to support care planning and coordination
  • Build positive relationships with school leadership to identify youth in need of support
  • Respond to emerging needs within the school setting with professionalism and empathy

Supportive Clinical Interventions

  • Conduct regular check-ins to reinforce coping skills and emotional stability
  • Educate youth and caregivers on diagnoses, medications, and treatment strategies
  • Use trauma-informed, youth-centered approaches to build trust and rapport
  • Recognize early warning signs of emotional distress and intervene appropriately

Documentation & Compliance

  • Maintain detailed and secure records of services, outcomes, and communication in the EHR
  • Ensure care plans and documentation meet organizational standards and HIPAA guidelines
  • Submit timely caseload inventory logs; assist with billing readiness by completing NPI registration and affiliation

Community Flexibility

  • While school-based, this role may require care delivery in homes or community settings when necessary to meet the needs of youth and families


Qualifications

  • Active RN license in the District of Columbia
  • Experience working with youth, particularly in school or behavioral health environments
  • Strong foundation in mental health care, trauma-informed practices, and care coordination
  • Excellent communication, documentation, and team collaboration skills
  • Familiarity with electronic health record systems
  • Completion of NPI registration and provider affiliation prior to start
  • Ability to work flexible hours and in varied settings as needed
