The Gardens at Monroe is looking for a full time Social Work assistant. Main responsibilities include care plan meetings and discharge planning coordination, conducting MDS assessments, and addressing the psycho-social needs of the patients. The ideal candidate will be caring and compassionate and able to work as part of an interdisciplinary team. LSW and one year experience in a SAR/Long Term Care setting preferred.
