Job Description:

As a Medical Social Worker at Roadrunner Home Healthcare, you will play a vital role in assisting patients and their families in navigating the emotional, social, and financial challenges of illness, aging, and recovery at home.

Key Responsibilities:

• Assess patients’ psychosocial status and support needs
• Provide emotional counseling and crisis intervention
• Assist families with long-term planning and healthcare decisions
• Connect patients to community resources, financial aid, or in-home support
• Collaborate with nurses, therapists, and physicians to coordinate care plans
• Educate patients and families on coping strategies, advanced directives, and more
Maintain accurate documentation in accordance with Medicare, Medicaid, and agency policies

Qualifications:

• Master’s Degree in Social Work (MSW) from an accredited institution
• Licensed in the state of New Mexico as an LCSW, LMSW, or equivalent
• Minimum 1 year of experience preferred
• Strong interpersonal, organizational, and communication skills
• Reliable transportation and valid driver’s license
