The VA Southern Oregon Rehabilitation Center & Clinics (SORCC) provides a variety of clinical programs with a wide range of outpatient services and treatments for Veterans in Geriatrics and Extended Care, Psychiatry, substance abuse, home based primary care, community care, residential, and various other ambulatory care programs. VA SORCC has a main campus and two Community Based Outpatient Clinics (Grants Pass and Klamath Falls). VA SORCC employs up to 102 Social Workers.

FUNCTIONS OR SCOPE OF ASSIGNED DUTIES:

The COMPACT Act Social Work Program Coordinator functions as a coordinator for care of Veterans at risk who have eligibility through the COMPACT Act. They serve as an advisor to facility leadership, program managers, and staff concerning the COMPACT Act and strategies for individual Veterans, the facility, and the Veteran community. This is accomplished through:

The COMPACT Act Social Work Program Coordinator is responsible for overseeing and enabling navigation between varying levels of care.
The COMPACT Act Social Work Program Coordinator is responsible for coordination between health care providers and care managers in direct care and community care.
The COMPACT Act Social Work Program Coordinator is responsible comprehensive transition planning.
The COMPACT Act Social Work Program Coordinator is responsible for PDE1 for unempaneled Veterans.
The COMPACT Act Social Work Program Coordinator is responsible for outreach to community providers and healthcare systems providing COMPACT care.

The COMPACT Act Social Work Program Coordinator provides outreach, manages administrative and clinical reviews, provides case management, and assists Veterans with navigation of the health care system.
The COMPACT Act Social Work Program Coordinator will be considered facility subject matter experts and will serve as a resource to Veterans, caregivers and staff in various areas including, but not limited to:
- Emergent Suicide Care and Suicide Prevention
- COMPACT Act Eligibility and Covered Services
- COMPACT Act Clinical Eligibility (mental health and related medical care)
- Pay Authorities
- Community Care Processes
- Billing Appeals

This position also includes all functions of a mental health advanced practice clinical social worker including and not limited to:

Screening/Assessment- The social worker will develop an assessment of the Veteran independently and/or in collaboration with the interdisciplinary treatment team, the Veteran, family members, and significant others.
Treatment Planning/Goa/ Setting - The social worker independently develops or contributes to the development of the mental health treatment plan and setting achievable treatment goals with the Veteran and/or family and with the interdisciplinary treatment team.
Documentation/Productivity- The social worker will complete documentation in compliance with appropriate documentation standards.
Referral to Service Providers - Throughout the course of treatment the social worker is the specialist on VA and/or community resources.
Resource Usage/Development- The social worker is responsible for using current social work resources and community social service programs and enhancing the content to the benefit of Veterans.
Advocacy- The social worker understands the challenges of bureaucracy and will act as an advocate when it serves the best interest of the Veteran and/or family.
Crisis Intervention - The social worker is experienced in making rapid assessments and developing crisis management plans for various types of crises including housing emergencies, psychiatric, suicide, and other high:-risk behaviors or situations.
Education/Health Promotion and Prevention - The social worker provides education related to VA and community resources, entitlements, Advance Directives/Living Wills, and will refer Veterans and/or families to the appropriate interdisciplinary team member or VA provider for identified health education needs.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to

to verify that the social work degree meets the 4 accreditation standards for a masters of social work.
Licensure: Must be licensed or certified by a state licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).

Grade Determinations:

Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs.
- (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
- (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
- (c) Ability to supervise multidisciplinary staff assigned to the program.
- (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
- (e) Ability to provide training, orientation, and guidance within clinical practice.

Preferred Experience: Knowledge of Suicide Prevention Strategies and Documentation. Experience in inpatient discharge planning and outpatient care coordination for complex patients preferred. Must have strong attention to detail and organizational skills.
