We’re seeking a compassionate, experienced Hospice Social Worker (MSW) to join our care team in the Virginia Beach area. In this role, you’ll provide emotional and resource support to patients and families, making 3–5 home visits per day based on patient needs and location. You’ll work autonomously, manage your own schedule, and help ensure patients receive age-friendly, dignified care in their homes.

Location: Virginia Beach, VA (Travel Required)

Employment Type: Full-Time, Monday–Friday, 8am–5pm (Contract)

Key Responsibilities:

• Provide psychosocial support to hospice patients and their families
• Visit patients in their homes across Virginia Beach, Norfolk, Chesapeake, and surrounding areas
• Attend interdisciplinary team (IDG) meetings to collaborate on patient care
• Manage documentation and follow-up referrals using HomeCare HomeBase (HCHB) EMR
• Refer families to community resources and coordinate volunteer support

What We’re Looking For:

• Master of Social Work (MSW) from an accredited program
• Virginia social work license (LMSW or LCSW)
• Valid driver’s license and reliable transportation
• Strong organizational skills and the ability to manage a high-volume caseload
• Previous experience in medical or hospice social work preferred
