The Nurse Care Coordinator - Outreach & Wound Care plays a critical role in delivering compassionate, community-centered care to individuals with complex wound care needs. This position is responsible for assessing, treating, and educating patients on wound management, while coordinating care between clinic-based and community-based services. As a key member of the harm reduction team, this role bridges gaps in healthcare access and helps reduce complications, promote healing, and enhance quality of life for underserved populations.
Work Environment:
This role involves both in-office and field-based work. Must be comfortable providing care in non-traditional settings, including outreach sites. Occasional evening or weekend hours may be required to meet client or community needs.

Essential Responsibilities:
- Assess and monitor wounds in diverse patient populations.
- Develop individualized treatment plans using evidence-based wound care practices.
- Monitor healing progress and make adjustments to treatment as needed.
- Educate patients and caregivers on proper wound care and preventive practices.
- Coordinate multidisciplinary care with physicians, behavioral health providers, and case managers.
- Ensure timely follow-up and continuity of care for assigned caseload.
- Document clinical findings and maintain accurate, up-to-date records in electronic health systems.
- Lead and participate in community outreach programs and events.
- Conduct educational workshops and in-home or community-based wound care support.
- Collaborate with external partners to extend services to high-need populations.
- Actively participate in quality improvement efforts
- Required:
- High School Diploma or equivalent
- Active LPN or RN license in good standing (Delaware or compact state)
- Minimum 3 years of nursing experience, with at least 1 year in wound care or community health
- Preferred:
- Associate's or higher in Nursing
- Wound Care Certification (WCC) or equivalent credential
- Strong clinical expertise in wound assessment and treatment
- Excellent communication and interpersonal skills
- Culturally competent, trauma-informed care approach
- Strong organizational skills with ability to manage multiple priorities
- Proficiency with medical documentation and electronic health records
- Commitment to harm reduction and healthcare equity principles
